  • To help us understand your situation, we are requesting that applicants record a video sharing your story and why you are a candidate for rent relief.  You may record from a phone or any other recording device and share a link from your phone, YouTube, Vimeo or any other video service.  Scroll down or click here for more information.

  • Upload bank statement showing current account amount
  • Valid Government issued ID

  • Applicant may be asked to upload documents verifying event such as job loss or termination letter, unemployment paperwork, death certificate, medical bill statements, doctor’s note, photos, and/or other documents

Tell Us Why You Need Temporary Rent Relief

Many people ask, how do I create a compelling “Share Your Story” video?  This is your opportunity to explain to us why you require rent relief, and the best way to do that is by telling the truth.  Your story must be no longer than 2 minutes and must include the following:

Video Must-Haves

  • Identify – Identification of all occupants in the household
  • Describe – A brief description of the event that is causing your rent relief request
  • Inspire – What you hope to achieve after receiving rent relief

Remember, it’s important to keep it concise and focus on the facts.  We are looking to verify your situation and understand your needs.  The clearer your message, the more likely you will be selected and approved.  As always, please keep it family-friendly.  We look forward to hearing your story.

How To Provide "Share Your Story" Video Link

Below are some of the ways you can share your “Share Your Story” video with us:

Once you have the link, copy and paste it into the application form field below titled “Provide ‘Share Your Story’ Video Link”.

What To Expect After Applying

  1. After submitting the form below, Relevium Foundation board members will review the initial application and video.
  2. If you pass the initial screening process / pre-approval round, an email will be sent to you with next steps, including a link to a secure Google Shared Drive
  3. You will upload additional verification documents, as described in the criteria.  You must also comply with any additional instructions.
    • Note: If you do not have a Google email account, you will be asked to create one when you click the secure Google Shared Drive link.  Instructions will be provided.
  4. Once verification documents are received, the board will conduct a final review to determine if rent relief is approved.
    • Approved applicants will be contacted via video call.
    • Declined applicants will be informed via email.

Payments

If approved, residents can receive up to 3 full or partial consecutive monthly rent payments within a 12-month leasing period.  Residents will be informed of the amount and the date at which rent payments will begin on their behalf.

Monthly payment(s) will be made by Relevium Foundation directly to the landlord or property management company on behalf of the resident.
